What is SMILE eye surgery?

SMILE (small incision lenticule extraction), or SMILE eye surgery, is the latest advancement in laser vision correction for the treatment of myopia (nearsightedness) and astigmatism. Sometimes called "flapless LASIK," it is a minimally invasive, FDA-approved procedure used to treat nearsightedness and astigmatism.

Mass Eye and Ear surgeons use the latest technology and equipment for laser vision correction surgery, including highly precise lasers. Mass Eye and Ear, Lexington is the only facility in New England that offers SMILE—one of the most gentle and precise vision correction procedures available.

How does SMILE surgery work?

During SMILE surgery, a highly precise and gentle ZEISS VisuMax femtosecond laser reshapes the cornea by removing a small amount of eye tissue. Unlike LASIK eye surgery, it does not create a corneal flap.

From start to finish, the entire procedure takes just a few minutes.

SMILE eye surgery is one of the gentlest vision correction procedures available. Most patients feel no pain during or after the procedure and are able to return to daily activities quickly. Risks and restrictions are similar to LASIK, though SMILE avoids flap-related complications.

Not everyone is a candidate for SMILE eye surgery. If your doctor doesn’t think SMILE is right for you, you may be eligible for other types of corrective surgery, such as LASIK eye surgery or PRK.

Patient shares her experience with SMILE eye surgery

Before undergoing SMILE, Adria Halasz couldn’t go anywhere without her contacts or glasses. “I was so nearsighted and dependent on visual aids,” she says. “I had a constant fear of not being able to see.”

Who is a good candidate for SMILE eye surgery?

If you have an active lifestyle or job, SMILE may be a better option for you than LASIK or similar procedures because there is no risk of dislodging a corneal flap.

You may be a candidate for SMILE if you:

  • Are at least 18 years old
  • Have a stable vision prescription for at least one year
  • Have nearsightedness between -1.00 and -10.00 diopters, and up to 3.00 diopters of astigmatism
  • Have healthy corneas and good overall eye health
  • Have realistic expectations about what SMILE can achieve

What should I know about recovery from SMILE eye surgery?

Recovery is quick after SMILE. Most people are able to return to work the next day. However, we recommend taking the day of surgery and the following day off so you can attend your follow-up appointment and ensure your eyes are healing properly.
